feat(cli): first-run wizard and auto Graphify index - #22

Ship a Greptile-shaped local CLI: `codeturtle` with no args prompts for GitHub token and Ollama model, then clone+index+dry-run review. Config lives in ~/.codeturtle/config.toml. Graphify is a default dependency. `codeturtle review owner/repo N` uses the same workspace. Offline goldens unchanged.
