feat(review): do not MERGE when change-unit coverage is low - #16

Use review_coverage (units_packed / units_total) in final policy. Empty KEEP + ratio below coverage_merge_min becomes COMMENT, not MERGE. Lockfile-only and supported+medium still win first. Low coverage never escalates to REQUEST_CHANGES. Final LLM is clamped to that decision.
