Summary

Migrate goal reminders from raw turn-start input prepending to the passive queued-context API added by letta-ai/letta-code#3773.

Before, goal mode inserted a user message at the front of the finalized turn input. On a turn carrying an approval continuation plus queued task notifications, that displaced the approval from index 0 and Core rejected the request with PENDING_APPROVAL.

After, goal mode contributes a typed passive context item. Letta Code owns composition and keeps the order approval -> goal context -> queued messages.
