Summary

This PR updates the code review state tracking file to reflect the latest automated review run, documenting newly identified issues and verified clean areas of the codebase.

Key Changes

• New filed issue [REVIEW] CLI: getVersion() uses require() in ESM module — --version and --help always display 0.0.0 #24: Identified a critical bug where getVersion() in src/cli.ts uses require() in an ESM module, causing --version and --help flags to always display 0.0.0 instead of the actual version 1.0.1
• New runner-up findings:
  • Permissions-Policy test failures due to merge regression (duplicate of [REVIEW] Permissions-Policy tests: 4 assertions broken by merge regression — main branch CI is red #15)
  • CSP maxScore misconfiguration where achievable maximum is 20 but maxScore is set to 30, artificially capping overall percentage at 90%
• Expanded clean areas verification:
  • Confirmed fetch.ts timeout implementation using AbortController + clearTimeout is correct
  • Verified analyzer.ts grade boundaries match documented grading scale
  • Validated index.ts analyze() correctly dispatches URL strings vs header objects
