Feature: Per-guild server credentials

Currently, all guilds on a bot instance share the same music server. This behavior should continue to be supported, but guilds should additionally be allowed to specify their own server type/details, so that one bot instance can support multiple guilds with multiple music servers.

This behavior should also be toggleable by the bot hoster, if they wish to either enforce a global music server, or the lack thereof.
