feat: add mobile card view for provider data table

- Show expandable cards on mobile instead of wide table
- Each provider card has header with photo, name, status, and action buttons
- Expanded view shows all fields with 'Use' buttons for applying values
- Table still shows on medium+ screens for desktop experience
