- 将 release.yml 与 promote.yml 合并为单一流水线，消除人工触发 workflow_dispatch 的操作断层
- 新增 production-gate job（environment: pypi + Required Reviewers）作为唯一人工审批关卡
- 新增 promote-and-publish job：审批通过后自动晋升 Release 元数据（prerelease=false）并发布到 PyPI
- 新增 update-changelog job：自动从 Release notes 生成 Changelog 条目并提交 PR → master
- 全流水线复用同一批 build artifacts，保证 TestPyPI 与 PyPI 包一致性
- 保留 hotfix 直接稳定版路径（prerelease=false → 直接发布到 PyPI）
- concurrency.cancel-in-progress 由 true 改为 false，防止发布中途被打断
- 删除已被合并的 promote.yml

流水线：Build → TestPyPI → Production Approval Gate ← 人工审批 → PyPI + Changelog PR
