- Refactored index code to be generic (supports both files)
- Normalized LEARNINGS.md entries to full timestamp format
- Auto-updates index on `ctx add learning`
- New `ctx learnings reindex` command for manual regeneration
- Converted inline bullet entries to structured format
Both DECISIONS.md and LEARNINGS.md now have scannable indexes
for AI agents with limited context budgets.
